This document outlines your rights as a patient and your responsibilities in supporting your care.

PATIENT RIGHTS

As a patient, you have the right to:

1. Respectful and Nondiscriminatory Care

Receive care that is respectful, considerate, and free from discrimination based on race, color, religion, sex, gender identity, sexual orientation, age, disability, national origin, or ability to pay, in accordance with applicable law.

2. Privacy and Confidentiality

Have your medical information kept private in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Georgia laws.

3. Information About Your Care

Receive clear and understandable information regarding:

  • Your diagnosis and treatment options
  • Expected outcomes and potential risks
  • Alternatives to proposed treatments

4. Participation in Care Decisions

Participate in decisions about your healthcare, including the right to:

  • Ask questions
  • Seek second opinions
  • Accept or refuse treatment, as permitted by law

5. Access to Medical Records

Access and obtain copies of your medical records in accordance with federal and state regulations.

6. Informed Consent

Receive information necessary to give informed consent before undergoing procedures or treatments.

7. Pain Management

Receive appropriate assessment and management of pain as part of your care plan.

8. Continuity of Care

Receive information about follow-up care, referrals, and ongoing treatment plans.

9. Complaints and Grievances

Voice concerns or complaints regarding your care or services without fear of retaliation.

Complaints may be directed to the Practice or to appropriate regulatory agencies.

10. Information About Practice Policies

Be informed of practice policies related to patient care, privacy, billing, and conduct.

PATIENT RESPONSIBILITIES

As a patient, you are responsible for:

1. Providing Accurate Information

Provide complete and accurate information regarding:

  • Medical history
  • Medications and allergies
  • Insurance and contact details

2. Following the Treatment Plan

Follow the treatment plan agreed upon with your healthcare provider or notify the Practice if you are unable or unwilling to do so.

3. Appointment Management

  • Arrive on time for scheduled appointments
  • Provide advance notice if you must cancel or reschedule
  • Understand that repeated missed appointments may affect scheduling privileges

4. Financial Responsibilities

Understand and comply with financial obligations, including:

  • Copayments
  • Deductibles
  • Coinsurance
  • Non-covered services

5. Respectful Conduct

Treat providers, staff, and other patients with courtesy and respect.

The Practice maintains a zero-tolerance policy for:

  • Abusive, threatening, or disruptive behavior
  • Harassment or intimidation

6. Safety and Infection Control

Follow safety and infection-control guidelines while in the Practice’s facilities.

7. Use of Electronic Services

Use patient portals and electronic communication responsibly and understand that such systems should not be used for emergencies.
